514 - Complex Ear Wax Removal off Eardrum and Anterior Recess of Ear Canal

Ear wax removal is necessary when ear wax build up becomes impacted. Symptoms causes by earwax blockage include: earache (otalgia), conductive hearing loss, blocked ear, vertigo, tinnitus, itchiness and irritation, occlusion etc. The client attended with wet and sticky medial ear wax stuck to his eardrum and lodged in the anterior recess of his ear canal which is the most difficult region in the ear to remove ear wax. The anterior recess is a hidden opening of the front canal wall near the eardrum. Quite often, the anterior recess is not visible with standard otoscopy or operating ENT microscope or head loupes. However, when it is usually visible with a wide field of view endoscope. The client had been using ear wax drops for around a week and also pushed his finger inside his ear to scratch an itch. Unfortunately, this lodged the wet and sticky ear wax directly onto his eardrum and inside his anterior recess. The external ear canal wall was very wet also with ear wax coating and staining it. I initially used a standard Zoellner 2mm diameter suction probe to remove the majority of the ear wax off the eardrum. I then attached an Fine End Gauge 18 suction tip and bent it to obtained access into the anterior recess. Without bending the Fine End, it would make contact with the bony anterior ear canal wall and be very uncomfortable for the client. Whenever ear wax is being removed off the eardrum, medially or the anterior recess, it becomes a complex procedure and the is risk of perforation the eardrum or making contact with the sensitive bony ear canal wall. Nonetheless, I managed to remove the lodged ear wax from the anterior recess much to the client’s delight. The ear canal itself still have some soft wet wax staining it. It is not possible to remove this, nor necessary, unless you use ear irrigation. Ear irrigation, however, introduces additional risks such as ear infection and perforation of the eardrum. Some ear wax in the ear is healthy and therefore causing unwarranted discomfort to the client to remove insignificant ear wax is not recommended. The remaining ear wax will naturally migrate out of the ear canal as the skin that lines the ear canal sheds and moves outwards, like a conveyor belt.
