This Video is dedicated to Pioneering work of dear friend, Harry Reich MD, who performed the first TLH in the world and paved the way for safe, simple laparoscopic Hysterectomy, the most common procedure in field of gynecology, we as physicians and all patients, in particular, who benefit from this technique, are indebted to his brilliant work. Our technique of lateral approach starts with a semi circular incision in anterior leaf of broad ligament after dividing both round ligaments, uterine vessels clearly identified by dissection, bladder flap developed to identify pubo-vesico-cervical fascia, sub-vascular space of shervin developed after incising posterior leaf of broad ligaments for safe suture ligation of uterine vessels over cervical cup of uterine manipulator away from ureters.. beside meticulous dissection technique all and all 7 sutures are used to accomplish the job completely. Which is clearly more economical in this day and age. Total blood loss for this patient was less than 50 cc, and procedure took about an hour with suspension, patient was operated on an outpatient basis and was discharged home 6 hours latter
