Ocarina of Time: Early Eyeball Frog (1.0, 1.1 only)

Thanks to Jarvitz for accidentally losing the Zora Tunic during Wodah's puzzle run, and Runnerguy for figuring out this much faster setup. I plan to upload my original method shortly. Basically, performing Collection Delay in certain ways will allow you to obtain the Eyeball Frog instead of the Zora Tunic It's a pretty simple setup. Put pretty much any item into the Adult Trade Item slot and have a shield equipped. Then when you talk to King Zora, simply hold R. What's more, if you somehow manage to mess up on delivering the Eyeball Frog, you'll get back the Prescription. Unfortunately, this appears to be only possible in the 1.0 and 1.1 versions of the game.
